The process begins with a Prescreen Application filled out completely and legibly by the person seeking treatment. Here is how to obtain and submit an application to the Wells House program:

- Hagerstown: 301-739-7748
We can send the form to you by mail or email. - You may also complete the application online by selecting the Admissions tab above, then clicking How to Apply for Admission, and scrolling down to the Wells House Prescreen Application.
- Please note: the online application must be completed using a laptop or tablet, as it is not compatible with mobile phones.
- Or, you can download the application from the website, complete it, and email it to admissions@wellshouse.org.
